The brief. Hospital lab and pharmacy events flowing through 42 Kafka topics into a NestJS consumer fleet — 1.2M events a day with full replay.
We built a Kafka spine partitioned by patient identifier so ordering is preserved per patient. Eight producer adapters publish normalised events; three NestJS consumer fleets fan out to clinical, billing and notification services. Every event carries a schema version and the registry rejects incompatible producers at deploy time. A Redis layer in front of the read API took p95 from 1.4s to 130ms. Integration failures became a queue metric instead of an outage — on-call pages dropped 92% in the first quarter and a new hospital now takes nine days instead of a quarter.
